1. Begin HHA Enrollment
Start your HHA enrollment. Payment and current enrollment will be verified. You will receive access to the application.

3. Physical & TB Clearance Form
Download the physical and TB clearance form. Must be completed by your healthcare provider.

Upload your completed application (CDPH 283D) and any required supporting documents securely.
