Complete the enrollment agreement for your CNA program.
3. Student Performance Fact Sheet
Download the physical and TB clearance form. Must be completed by your healthcare provider.
4. Physical Examination and TB Clearance Form
Download the physical and TB clearance form. Must be completed by your healthcare provider.
5. Live Scan Fingerprinting Form
Upload your completed application (CDPH 283D) and any required supporting documents securely.
6. Student Acknowledgements & Agreements
Upload your completed application (CDPH 283D) and any required supporting documents securely.
